When Long-Term Care is the Best Option

When is the right time?  That decision can be a hard one for not only you, but your loved one. It’s important to remember that the right skilled nursing facility can be an enriching and rewarding environment for your loved one and for you as their caregiver. Health and safety concerns are the main reason why someone may need to be admitted into long-term care:

Care Team Meetings

Every family is invited to meet regularly with our team of highly trained professionals to review and participate in the resident’s treatment and plan of care. The team consists of a physician, psychiatrist/psychologist, nurses, certified nursing assistants, social workers, therapists, recreation leaders and dietitians. Members of this team are specialty trained and dedicated to the care of the long term population.

Hospice/Palliative Care Program

We offer end-of-life care by coordinating with Hospice and Palliative Care agencies for residents in need of comfort care. Guided as much as possible by each patient’s personal preferences and care goals, AOH works to bring meaning and fulfillment during the final stage of life. Our program focuses on managing pain, while providing emotional, social and psychological support to the resident and family members.
